[No.1815] 多巣性頭蓋内および頭蓋外動脈瘤を伴う末梢眼動脈瘤：症例報告紹介

清澤のコメント：そんなに多いものかと思いますが、 EB Dinca 2017によれば、眼動脈瘤は全脳動脈瘤の 5% を占め、くも膜下疾患に関連する罹患率と死亡率の重要な原因となっているともいわれます。画像を検索してみると、「内頚動脈と眼動脈の分岐部領域」の動脈瘤Aneurysms of the ophthalmic segment in: Journal of Neurosurgery Volume 72 Issue 5 (1990)）という事のようです。今回、眼動脈の動脈瘤に関するケースレポートとレビューが出版されました。眼動脈動脈瘤という概念があるという以上の情報は今のところ無い模様です。その中に東北大学で当時の小川助教授と脳外科の冨永悌二先生が書いた論文（私も共著に加えていただきました）も引用されていました。懐かしい時代の論文です。

Peripheral Ophthalmic Artery Aneurysm Associated with Multifocal Intracranial and Extracranial Aneurysms: Case Report and Literature Review
概要
末梢眼動脈瘤はまれな疾患です。我々は関連文献を検討し、デジタルサブトラクション血管造影法で診断された、複数の頭蓋内および頭蓋外動脈瘤を伴う眼窩内眼動脈全体を伴う紡錘状動脈瘤の症例を報告する。患者は圧迫性視神経障害による不可逆的な失明を患い、メチルプレドニゾロンの静脈内投与を3日間試しても改善しなかった。自己免疫検査は正常でした。根本的な原因は不明です。
Abstract
Peripheral ophthalmic artery aneurysm is a rare disease entity. We review the relevant literature and report a case of fusiform aneurysm involving the entire intraorbital ophthalmic artery in association with multiple intracranial and extracranial aneurysms, diagnosed on digital subtraction angiography. The patient suffered irreversible blindness secondary to compressive optic neuropathy which did not improve after a 3-day trial of intravenous methylprednisolone. Autoimmune screen was normal. The underlying cause is unknown.

    • 眼窩内眼動脈の未破裂動脈瘤の稀な症例が報告されており、動脈瘤の切除が成功し、動脈瘤の質量効果によって引き起こされる術前の進行性徴候が改善されました。この稀な存在の外科的管理について、外頸動脈から眼動脈の側副循環に特に注意を払って説明します。
    Abstract
    A rare case is presented of a nonruptured aneurysm of the intraorbital ophthalmic artery in which successful resection of the aneurysm resulted in improvement of preoperative progressive signs caused by the mass effect of the aneurysm. The surgical management of this rare entity is discussed with special attention given to the collateral circulation of the ophthalmic artery from the external carotid artery.
